The chart below indicates the state-required health evaluations and immunizations required as the student enters the corresponding grade level. All physical and dental forms are due by October 15 of the current school year. On the first day of school, unless the child has a medical or religious/philosophical exemption, a child must have at least one dose of the required vaccinations or risk exclusion.
