Action Plans provide specific plans of care directed by your child's physician for conditions such as asthma and food allergies. A new Action Plan is required EACH school year and must be signed by the physician AND a parent/guardian. A separate medication dispensing form is not needed if medications are on the Action Plan unless the student will self-carry (epi pens and asthma inhalers only).
